Acute ischemic stroke is a leading cause of death and major disability worldwide. Approximately 50% strokes are caused by atherothrombotic occlusion the cerebral arteries, antiplatelets mainstay secondary preventative treatment. Aspirin beneficial if given early, short‐term treatment using aspirin clopidogrel increasingly used for patients with intracranial atherosclerotic disease, minor stroke, or transient attack. However, up to continue have recurrent vascular events, which may be partly attributable resistance clopidogrel. Although precise mechanisms unknown, clinical genetic factors associated bioavailability binding target receptors implicated. This narrative review begins concept in attack, potential including polymorphisms, an overview platelet function measures limitations. We conclude highlighting practical issues management andclopidogrel emerging interest ticagrelor, prasugrel, cilostazol as well directions future trials attack acute stroke.
